As a Clinical Nurse - CHIP, you'll help people with complex health needs move safely from hospital to home and community-based care. Working across disciplines and services, you'll coordinate care, build strong partnerships and contribute to outcomes that improve wellbeing and independence.
Every day, you'll contribute by:
Coordinating personalised care plans that support safe, timely transitions and positive patient outcomes.
Building strong relationships with patients, families and healthcare partners to navigate complex care needs.
Using advanced clinical judgement to identify risks, solve problems and improve continuity of care.
Contributing to service improvements, mentoring colleagues and promoting high-quality care practices.

Current registration as a Registered Nurse.
Experience coordinating care and managing complex clinical situations.
Strong communication, stakeholder engagement and problem-solving skills.
Commitment to quality improvement, professional growth and patient-centred care.
You'll stand out from the crowd if you have postgraduate study relevant to nursing practice and/or experience working across community, rural or multidisciplinary healthcare settings.
