Job Description:
We are seeking a qualified, experienced Support Worker to provide NDIS-funded supports to a participant located in SpringVale , Victoria.
This role involves delivering person-centred, strengths-based support including assistance with personal care, to help the participant safely engage in daily living activities, community access, and social participation, while upholding dignity, choice, and control at all times.
Key Responsibilities:
- Personal care support including assistance with hygiene, grooming, dressing, and toileting (as required)
- Household tasks including light cleaning and laundry related to the participant
- Transport support to appointments, community activities, and shopping (using a reliable vehicle)
- Social and community participation, supporting inclusion, relationship-building, and skill development
- Support communication needs using appropriate strategies and guidance from family and allied health professionals
- Follow established routines, behaviour support strategies, and risk assessments
- Maintain clear and accurate communication with family and relevant stakeholders
- Ensure participant safety, privacy, and wellbeing at all times
Essential Requirements:
- Certificate III in Individual Support (Disability) or Certificate IV (or equivalent) (essential)
- Current NDIS Worker Screening Clearance
- Current National Police Check
- Valid Working With Children Check (VIC)
- Current First Aid Certificate including CPR
- Current Manual Handling Certificate
- Valid Australian driver’s licence and reliable transport
- Full Australian working rights
- Demonstrated understanding of NDIS Practice Standards, Duty of Care, and Child Safe requirements
