What your day could look like
As an Aboriginal and Torres Strait Islander Advanced Health Worker, you'll play a vital role in improving health outcomes and strengthening trust between communities and our health services. From coordinating care across diverse health needs to empowering people through education and advocacy, you'll work alongside a collaborative team to deliver culturally respectful, person-centred care.
Every day, you'll contribute by:
Coordinating care and developing tailored plans that improve access, continuity, and health outcomes
Supporting and advocating for patients and families, ensuring their voices are heard and needs are met
Delivering health promotion, education that empowers individuals to make informed decisions
Building strong relationships with community groups, healthcare partners to create lasting impact
Let's talk about you!
You bring both cultural insight and advanced capability to your work. You understand the importance of trust, respect, and connection, and you use your skills to make a real difference in people's lives. You're someone who leads with compassion, communicates with integrity, and takes pride in supporting your community through meaningful care and advocacy. You're driven to create better outcomes, not just deliver services, and you value working in a team that shares this purpose.
Here's what will help you thrive in this role:
Qualification in Aboriginal and Torres Strait Islander Primary Health Care (Cert IV or Diploma)
Strong understanding of Aboriginal and Torres Strait Islander health, cultures, and community needs
Experience coordinating care, supporting patients, and working collaboratively with diverse stakeholders
Ability to communicate clearly, build relationships, and contribute to a positive team culture
You'll stand out from the crowd if you have experience mentoring others and delivering community-based health programs.
Most importantly, you'll be passionate about improving health equity and committed to making a difference in the lives of Aboriginal and Torres Strait Islander people, while living our values of compassion, integrity, dignity, innovation, and courage. Together, you'll help create stronger communities and better health outcomes.
