About the Role :
We are seeking a compassionate and dedicated Family Support Worker to join our team in a fixed-term, part-time position (0.8 FTE, four days per week) based in Cooloola . This contract is until 8 January 2027 . In this role, you will provide therapeutic support to individuals, families, and groups experiencing social, emotional, and financial challenges. Working within a child-focused and whole-of-family framework, you will empower clients to build resilience, strengthen family relationships, enhance their wellbeing, and increase their connection to community supports and services.
You will play a key role in ensuring the best interests of children and families remain at the centre of service delivery, providing practical assistance, information, referrals, and ongoing support tailored to individual needs. This role requires a professional who can build positive relationships with clients, maintain confidentiality, and deliver services with empathy, respect, and a non-judgemental approach.
Responsibilities:
- Conduct intake assessments and determine client eligibility through phone, online, and face-to-face interactions.
- Complete psychosocial assessments, risk assessments, and safety plans for individuals and families with complex support needs.
- Develop, implement, and review client support plans, providing appropriate referrals and coordinated services.
- Deliver outreach support where required, including home visits, school visits, and community-based service delivery.
- Maintain accurate case notes, client records, reports, and documentation in accordance with organisational and legislative requirements.
- Provide client-centred support to individuals, couples, and families to strengthen relationships, parenting capacity, and overall wellbeing.
- Advocate for client rights and collaborate with government, statutory, and community agencies to achieve positive outcomes.
- Manage an active caseload, conduct outcomes-based assessments, and assist with case, court, progress, and termination reports.
- Facilitate and contribute to community education programs, workshops, and initiatives that promote family wellbeing and resilience.

Qualifications, Credentials and Other Requirements
- Certificate 4 or Diploma in Children, Youth and Family Intervention, community services or similar (this is an essential requirement of the role)
- Current National Police Check
- Current Blue Card
- Current Queensland Driver Licence
- Ability and willingness to travel within the service region.
