Care Partner – Support at Home and CHSP (Polish and/or Slavic Language Preferred)
PAWA Community Care Inc is looking for an experienced Community Based Care Partner to join our team for a permanent position.
We are a not-for-profit incorporated association with over 50 years’ experience in provision a range of services to frail aged people in Metropolitan Sydney. We specialise in supporting people from diverse multicultural backgrounds.
Key responsibilities
You will be a part of our multicultural care management team.
Your goal will be working, as part of our team, towards ensuring that assessed and delivered care plans, individual budgets and strategies achieve optimum outcomes, standards, flexibility, responsiveness for participants within legislative and regulatory requirements of the SAH and CHSP programs, strengthen aged care standards, and other relevant legal frameworks.
As a Care Partner you will be are responsible for:
- On-Boarding new clients – Support at Home and CHSP; and assessing their needs and services required
- Ensuring all client documentation is complete, correct and forwarded to the appropriate area for filing
- Assessing client’s needs, developing, monitoring and reviewing Person Centred Care Plans for all clients
- Assisting clients by referring them to providers that can provide a different range of services and liaising with these providers such as: Allied health providers, Australian Government Organisations, community groups and agencies and private organisations to enhance delivery of our community services.
- Ensuring PAWACC develops and maintains a supportive and positive relationship with clients and their families and advocates.
- Working with Support at Home and CHSP consumers to ensure that they obtain maximum benefit from the Sah subsidy including guidance and assistance with eligible expenditure of their package funds.
- Managing client’s budgets and preparing reports and submissions for funding and resources including reviews for package upgrades and support review plans for disability funding.
- Working with clients to promote independence and reablement.
- Managing one to one relationship with clients and their registered supporters
- Providing information and education to clients, families and advocates about available services including government funded services.
- Approving appropriate client requests.
- Working collaboratively within PAWACC to ensure continuous improvement.
- Establishing appropriate links with the Community, other stakeholders
- Participate in Support Worker performance management and development.

- Working Knowledge of the Regulations, standards, frameworks and codes.
Qualifications and Experience
The following qualifications and experience are highly desirable for the position:
- Diploma of Community Services – Case Management
- Cert IV in Aged Care or Disability Care
- Experience working in the Aged Care or Disability sector
- Ability to speak Polish and/or other Slavic language
- Demonstrated knowledge and previous experience as a Care Coordinator or similar role
- Current Police Check and Working with Children Check
- Current First Aid Certificate
- Entitlement to work in Australia
Work environment
· Expected to travel to client home. Must be able to drive the Greater Sydney region
· Hybrid work from office/home arrangements after probationary period
· Rostered after hours on-call
Pay: $80,000.00 – $100,000.00 per year
