Due to continued growth, we are seeking a dedicated and compassionate Care Coordinator / Scheduler to join our Eastwood office team.
About the Role
As a Care Coordinator, you will play a key role in ensuring our clients receive consistent, high-quality support services. You will work closely with clients, families, support workers, and other stakeholders to coordinate care, manage rosters, and support service delivery.
Key Responsibilities
- Build positive and trusting relationships with clients and their families.
- Support clients to achieve their individual goals and care outcomes.
- Coordinate and monitor service delivery to ensure client needs are met.
- Maintain accurate and timely client records and documentation.
- Liaise with clients, families, and support staff regarding service requirements.
- Prepare and manage staff rosters based on client needs, worker availability, and qualifications.
- Coordinate roster changes and communicate updates to relevant stakeholders.
- Assist with day-to-day operational and administrative tasks.
About You
To be successful in this role, you will have:
- Excellent communication skills in both Mandarin and English (written and spoken).
- Previous experience in the aged care, disability, community services, or CHSP sector.
- Strong customer service and relationship management skills.
- Excellent organisational, time management, and problem-solving abilities.
- Strong computer and administrative skills with attention to detail.
- A caring, reliable, and compassionate approach to supporting others.
- Relevant qualifications and/or experience in community services, health, aged care, disability, or a related field.
Essential Requirements
- National Police Check (or willingness to obtain).
- Valid working rights in Australia.
- Current COVID-19 vaccination status.
- Ability to provide professional references.
