We are looking for a compassionate and motivated Case Manager to join our Eastwood office on a full-time basis. In this role, you will work closely with clients receiving Support at Home (SaH) Packages, ensuring they receive quality, person-centred services that help them live independently at home.
What you’ll do:
- Manage a portfolio of Support At Home clients.
- Develop, review and monitor care plans to meet client goals.
- Provide advice and guidance to clients and their families.
- Coordinate the delivery of all services by Support Staff, including scheduling/rostering
- Monitor package budgets and ensure compliance.
- Work Health and Safety issues/Risk Management
- A mixture of being in the office and being out in the community visiting clients
What you’ll bring:
- Qualification in Nursing, Community or Human Services, Case Management, Psychology, Social Work, Allied Health or Rehabilitation Case Management.
- Experience in case management and aged care.
- Strong knowledge of Support at Home funding and Aged Care Quality Standards.
- Great communication and problem-solving skills.
- Excellent documentation and computer skills
- Police Check / NDIS Worker Screening Check (or willing to obtain).
- Unrestricted current Australian driver licence and reliable car.
