As a clinician within the Residential Support Program (RSP), you provide specialist mental health assessment, intervention, consultation, and support to older adults experiencing persistent mental illness and/or behavioural and psychological symptoms of dementia (BPSD). Working as part of a multidisciplinary, recovery-oriented team, you partner with consumers, carers, residential aged care providers, and community services to prevent accommodation breakdown, enhance independence, and improve quality of life across community and residential settings.

Conduct comprehensive mental health and behavioural assessments, develop care plans, and deliver evidence-based interventions for older adults with complex mental health needs and/or BPSD.

Provide specialist consultation, education, and behavioural support to carers, residential aged care staff, and community service providers to promote person-centred care and prevent placement breakdown.

Deliver recovery-oriented case management and discipline-specific clinical expertise to support consumers across community and residential aged care settings.

Collaborate with multidisciplinary teams, external providers, and key stakeholders to coordinate care and achieve optimal consumer outcomes.

Support consumers and carers to enhance independence, wellbeing, and engagement with appropriate mental health and aged care services.

Contribute to continuous service improvement, multidisciplinary teamwork, and the delivery of high-quality specialist aged mental health care.
Classifications for this position will range from RPN Grade 4 NP75 - NP77 ($121,274 - $130,603 per annum pro rata)
Relevant postgraduate qualifications
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
5 years post qualification clinical experience with older adults including consumers with mental illness and dementia, complex needs and support to their carers’.
Highly developed interpersonal and communication skills
Demonstrated knowledge and understanding of dementia and BPSD and mental health issues.
Demonstrated expert knowledge of relevant legislation and current national and state directions in the delivery of aged and mental health services.
Strong understanding of evidenced based practice
Clear motivation to support clinical practice and professional practice development.
Current Victorian Driver’s License
A valid Nationally Coordinated Criminal History Check, or willing to obtain

SVHM Mental Health service consists of a variety of inpatient, specialist and community programs and utilises a strengths/recovery orientated approach.The St Vincent’s Aged Mental Health Service comprises the Aged Psychiatry Assessment and Treatment Team (APATT), Residential Support Team (RSP) and Healthy Ageing Service (HAS), Normanby Unit (20 bed acute mental health inpatient care) and continuing care via the Psychogeriatric Nursing Homes (PGNHs).
