Role purpose:
AVWA is a leading provider of aged care and disability support services, delivering high-quality care to elderly clients under the Home Care Package (HCP) program and participants under the National Disability Insurance Scheme (NDIS). Our mission is to enhance independence, improve daily living skills, provide rehabilitation support, and ensure safe, accessible, and functional home environments for our clients.
The Occupational Therapist (OT) plays a vital role in supporting aged care clients and NDIS participants by conducting home assessments, providing rehabilitation support, prescribing assistive technology, and collaborating with multidisciplinary teams to improve clients' safety, accessibility, and overall well-being. The OT will ensure services align with Aged Care Quality Standards and NDIS Practice Standards, participate in clinical audits, quality improvement initiatives, and professional development programs, and contribute to ongoing service improvements.
Key Responsibilities:
Aged Care Clients (HCP Program)
- Conduct home assessments to recommend modifications for safety, accessibility, and functionality.
- Develop individualised therapy plans to improve daily living skills and promote independence.
- Assess and prescribe assistive technology, mobility aids, and adaptive equipment.
- Train clients, family carers, and care workers on the correct and safe use of assistive technology, mobility aids, and adaptive equipment.
- Provide rehabilitation support for individuals recovering from injuries, surgeries, strokes, and degenerative conditions to restore functional independence.
- Implement therapy interventions to improve mobility, strength, coordination, and fine motor skills.
- Develop and implement falls prevention strategies and educate clients and family carers on risk reduction.
- Support clients with cognitive impairments (e.g., dementia, Parkinson’s disease) by implementing strategies to improve memory, problem-solving, and functional independence.
- Collaborate with Care Managers, Physiotherapists, and Care Workers to ensure integrated and holistic care planning.
For NDIS Participants
- Conduct functional capacity assessments to support NDIS planning and goal setting.
- Assess and recommend home modifications to improve accessibility and functionality.
- Develop and implement individualised rehabilitation programs for NDIS participants recovering from injuries, surgeries, strokes, and degenerative conditions.
- Assess and prescribe complex assistive technology and submit applications for NDIS funding approval.
- Train NDIS participants, family carers, and support workers on the correct and safe use of assistive technology, mobility aids, and adaptive equipment.
- Provide skill development programs to help clients gain confidence in daily living tasks such as cooking, dressing, and mobility.
- Implement therapy interventions to enhance fine motor skills, sensory processing, and functional independence.
- Work collaboratively with Support Coordinators, Plan Managers, and allied health professionals to optimise service delivery.
- Complete clinical reports, progress notes, and NDIS funding applications as required.
Home Assessments & Multidisciplinary Collaboration
- Conduct home safety and accessibility assessments and recommend modifications to improve functionality and reduce risks.
- Collaborate with Physiotherapists, Care Managers, Support Coordinators, Care Workers, and Support Workers to ensure a holistic approach to client care.
- Liaise with builders and home modification specialists to ensure effective and practical solutions for clients.
Compliance, Quality Improvement & Professional Development
- Follow Aged Care Quality Standards and NDIS Practice Standards in clinical care and service delivery.
- Participate in clinical audits, quality improvement initiatives, and registration audits to ensure compliance and service enhancement.
- Maintain accurate and up-to-date clinical documentation in line with HCP and NDIS requirements.
- Attend team meetings, training sessions, and professional development programs to stay updated on industry best practices.
- Keep informed about changes in aged care and disability policies, funding models, and evidence-based practices.
- Ensure compliance with all regulatory requirements, professional standards, and ethical guidelines.
Requirements:
- Bachelor degree in Occupational Therapy.
- Current AHPRA registration as an Occupational Therapist.
- Experience working with aged care clients (HCP) and/or NDIS participants.
- Strong understanding of Aged Care Standards, NDIS Practice Standards, and funding frameworks.
- Experience in functional assessments, home modifications, assistive technology, and therapy planning.
- Ability to train and educate clients, family carers, and care workers on assistive technology and equipment.
- Experience in rehabilitation therapy for injuries, surgeries, strokes, and degenerative conditions.
- Proficiency in conducting home assessments and recommending modifications for safety and accessibility.
- Excellent communication, problem-solving, and report-writing skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Valid driver’s license and willingness to travel for home visits.
- NDIS Worker Screening Check (or willingness to obtain).
Good to have:
- Experience with complex NDIS functional assessments and reports.
- Training or certification in home modifications and assistive technology prescription.
- Experience working with clients with neurological conditions (e.g., stroke, Parkinson’s, autism, intellectual disability).
- Proficiency in English and Vietnamese language skills.
