Role Description The Intake Officer / Support Coordinator (Level 3 – Specialist Support Coordination) is a full-time, on-site role based in Clyde, VIC. This role is responsible for managing intake processes, assessing participant needs, and coordinating supports in line with NDIS plans and goals. Key day-to-day tasks include conducting initial interviews, gathering documentation, developing and implementing coordination plans, and liaising with participants, families, service providers, and other stakeholders. The role involves monitoring service delivery, troubleshooting issues, advocating for participants, and ensuring supports are responsive, culturally safe, and aligned with NDIS guidelines. The Intake Officer / Support Coordinator maintains accurate records, prepares reports, and contributes to continuous improvement of service quality and client experience.
Key Responsibilities
- Manage participant enquiries, intake assessments, and onboarding processes.
- Develop and maintain service agreements and participant documentation.
- Coordinate supports and connect participants with appropriate services and community resources.
- Provide Level 2 and, where appropriate, Level 3 Specialist Support Coordination for participants with complex needs.
- Support participants to understand and effectively utilise their NDIS plans and funding.
- Collaborate with families, carers, providers, and allied health professionals to achieve positive participant outcomes.
- Monitor service delivery, identify barriers, and implement strategies to address challenges.
- Maintain accurate records, case notes, and reports in line with organisational and NDIS requirements.
- Promote participant choice, control, independence, and capacity building.
Skills & Experience
- Demonstrated experience in NDIS Support Coordination and participant intake.
- Strong understanding of NDIS funding, legislation, practice standards, and compliance requirements.
- Experience managing service agreements, onboarding processes, and participant documentation.
- Previous experience in Specialist Support Coordination (Level 3) is highly regarded.
- Ability to work effectively with participants experiencing complex circumstances and support needs.
- Excellent communication, stakeholder engagement, and relationship-building skills.
- Strong organisational, time-management, and problem-solving abilities.
- Proficiency in case management systems and maintaining accurate documentation.
- Relevant qualifications in Community Services, Disability, Social Work, Psychology, or a related field are desirable.
