What you'll be doing
As a Chronic & Complex Care Endorsed Enrolled Nurse, you will provide direct nursing care under the supervision of a Registered Nurse, helping clients manage chronic and complex health conditions within a community setting. You will contribute to positive health outcomes through assessment, care delivery, documentation and effective communication with clients, carers and healthcare professionals.
- Deliver safe, evidence-based nursing care in line with individual care plans and scope of practice
- Monitor client health status, undertake assessments and promptly escalate clinical concerns
- Support clients with activities of daily living while promoting independence and wellbeing
- Maintain accurate and timely clinical documentation to support quality care outcomes
- Partner with Registered Nurses and the wider healthcare team to provide coordinated, person-centred care

Essential Criteria:
- Registration: Current Endorsed Enrolled Nurse division 2 Australian Health Practitioner Regulation Agency (AHPRA)

3. Demonstrated effective communication skills both written and verbal.
4. Effective interpersonal skills showing initiative and leadership.
5. Ability to work as a member of a multidisciplinary team
6. Commitment to continuing professional development
7. Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
8. Current Drivers Licence valid within Australia and willingness to use for work purposes
