Lead, Innovate, Transform: Chronic Liver Disease Nurse Consultant
Join a rewarding role where you will coordinate an evidence-based preventative care program for people living with chronic liver disease, helping to improve outcomes through early screening, assessment and intervention. As an expert clinician, you will provide specialist leadership and work autonomously to deliver high-quality, patient-centred care across hospital and community settings. You will play a vital role in bridging the gap between acute and community services, supporting care planning, rapid response pathways, and hospital avoidance initiatives. Working closely with multidisciplinary teams, you will help drive innovative programs and service improvements that enhance patient outcomes and the delivery of chronic liver disease care.
About you:
Effective leadership skills including highly developed skills in communication, problem solving, conflict resolution and negotiation skills
Registered Nurse and or Midwife with at least 3 years post registration experience
Demonstrated experience and competence in Chronic Disease Management
Experience in the acute care transitions to community sectors
Ability to prioritise workload and meet set timelines
