As a registered NDIS provider expanding into home care services, we support people living with disabilities, older Australians, and individuals who require assistance to remain safely in their homes. Our person-centred approach focuses on promoting independence, enhancing quality of life, and delivering compassionate care tailored to each person's goals and needs.
Key responsibilities include:
- Responding to enquiries
- Coordinating referrals
- Verifying eligibility
- Preparing service agreements
- Scheduling initial assessments
- Supporting client onboarding
- Maintaining accurate records
- Liaising with care coordinators and clinical staff to ensure services commence smoothly
- Ensuring all documentation complies with the Aged Care Act, Support at Home Program requirements, organisational policies, and privacy legislation
To be successful, you will have:
- Relevant Industry Experience - Previous experience in the NDIS, home care or community services sector is preferred.
- Relevant qualification - In aged care, disability, community services or a related field.
- Nursing Background - Nursing qualifications or previous nursing experience is preferred.
- Excellent Communication Skills - Strong verbal and written communication, with a friendly, approachable and professional manner and the ability to build positive relationships with clients, families and stakeholders.
- Strong Administration & Computer Skills - Confident using computers and managing administrative tasks accurately and efficiently.
- Organisation & Time Management - Highly organised, detail-oriented and able to manage competing priorities and multitask effectively.
- Adaptability & Independence - Comfortable working in a fast-paced environment, taking initiative and working independently when required.
- Person-Centred Care & Teamwork - Compassionate, empathetic and patient, with a strong commitment to high-quality, person-centred care and a positive, team-focused approach.
- Reliability & Professional Commitment - Reliable, proactive and committed to the values and mission of Aussie Life Care, with flexibility to travel when required (company vehicle provided).

Pay: $75,000.00 – $90,000.00 per year
