Allegiance Heart & Home Care is seeking a reliable, compassionate and highly organised Support Worker to provide consistent in-home and community support to an older male client in Padstow, NSW.
This is a rewarding opportunity for a Support Worker who understands the importance of building trust, providing emotional reassurance and helping a client remain organised, connected and independent at home.
The successful applicant will work regular three-hour shifts on:

Monday: 11:30 am–2:30 pm
- Tuesday: 11:30 am–2:30 pm
- Wednesday: 11:30 am–2:30 pm
- Friday: 11:30 am–2:30 pm
Consistency is essential. We are looking for someone who can commit to the ongoing roster and gradually develop a positive, respectful and trusting relationship with the client.
You will support the client with:

Medication prompting in accordance with the care plan
- Organising appointments, reminders and daily tasks
- Entering appointments into his mobile phone and calendar
- Assisting with phone calls, emails, paperwork and follow-up tasks
- Helping organise his journal, documents and home office
- Basic support with mobile phones, computers and applications
- Accompanying him to medical and allied health appointments
- Shopping, errands and short community outings
- Social support, companionship and emotional reassurance
- Encouraging personal care, meals, hydration and daily routines
- Supporting him to reconnect with meaningful interests and family activities
- Monitoring and reporting changes in his health, mobility, memory, mood or continence
- Maintaining clear, accurate and timely progress notes
The ideal candidate will be:

Honest, trustworthy and dependable
- Compassionate, patient and non-judgmental
- Calm and reassuring when supporting someone experiencing anxiety or memory difficulties
- Confident using smartphones, calendars, email, applications and computers
- Highly organised and able to follow up appointments and outstanding tasks
- Comfortable providing social support and community access
- Able to maintain professional boundaries and confidentiality
- A clear communicator with strong written progress-note skills
- Willing to work collaboratively with the Case Manager, nurses and allied health professionals
- Available for all four regular shifts each week
Experience supporting older people with cognitive impairment, depression, anxiety, mobility concerns or complex health needs will be highly regarded.
Applicants must have:

Certificate III or IV in Individual Support, Aged Care, Disability or a related qualification
- Current First Aid and CPR certificates
- Current National Police Check
- NDIS Worker Screening Check
- Working with Children Check, where required
- Valid Australian driver’s licence
- Reliable, registered and insured vehicle
- Current comprehensive motor vehicle insurance
- Evidence of Australian working rights
- Availability for Monday, Tuesday, Wednesday and Friday from 11:30 am to 2:30 pm
