The Nurse Unit Manager Level 2 (NUM2) Triple I leads a highly visible and evolving central intake service that plays a critical role in connecting consumers, carers and referrers with community health services across South Western Sydney Local Health District. As the operational and clinical leader of Triple I, the NUM2 oversees referral management, intake and screening processes, service performance, workforce capability and quality improvement, ensuring referrals are managed efficiently and directed to the most appropriate service in a timely manner.
Triple I is a key component of the District's central intake model, providing a single point of access for referrals and supporting consistent, streamlined pathways into community health services. The NUM2 leads a team of nursing and administrative staff responsible for referral management, consumer and referrer support, service coordination, and the application of clinical judgement to referral prioritisation and escalation processes.Working closely with community services, hospital teams, primary care providers and district stakeholders, the NUM2 drives innovation, service improvement and operational excellence. The role contributes to improved access to care, reduced delays in service commencement, enhanced patient flow and safer transitions across the healthcare system. We are seeking an experienced and forward-thinking nursing leader with a passion for service improvement, patient access and operational excellence. The successful candidate will bring strong leadership experience in community health, intake, virtual care or service coordination environments, with a proven ability to lead and develop high-performing teams through periods of growth and change.
You will be highly organised, adaptable and solutions-focused, with demonstrated experience managing competing priorities, service demand and workforce performance in a fast-paced environment. Strong communication and relationship-building skills are essential, as you will work collaboratively with clinical services, hospital teams, primary care providers and district stakeholders to strengthen referral pathways and improve access to care.
As a leader, you will be committed to fostering a supportive and inclusive team culture, recognising the importance of staff wellbeing, engagement and development. You will bring the skills and emotional intelligence required to lead a nursing and administrative workforce through ongoing change, providing clear direction, mentoring and support while building capability and resilience across the team.
The ideal candidate will have a sound understanding of clinical governance, risk management and quality improvement, coupled with the ability to use data and insights to drive service performance and innovation. Most importantly, you will be committed to delivering a patient-centred approach to central intake, ensuring consumers, carers and referrers experience timely, streamlined and equitable access to community health services.
Selection Criteria

- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA). Relevant tertiary qualifications or equivalent work experience or a combination of study and experience deemed appropriate by SWSLHD.

Describe a time when you led a team through a period of substantial change. How did you maintain staff engagement and wellbeing while delivering required service outcomes, and what were the results?
