The position
As a Care Partner, you are at the heart of our mission, empowering older adults to remain independent, safe, and connected within their own homes and communities. Your days will be filled with meaningful interactions as you:

Bring your care coordination skills, client-focused mindset, and passion for making a difference. As a Care Partner, you'll work closely with clients and their support networks to deliver personalised services that enhance wellbeing, independence, and quality of life.
What’s required
- Diploma qualification in a relevant field or higher (e.g. Advanced Diploma, Bachelor degree or above)
- Experience in case management, care coordination or client services within aged care, community services or a similar environment
- Experience completing assessments, developing care plans and supporting clients with complex needs
- Ability to manage consumer budgets and support services effectively
- Sound computer skills, including Microsoft Office and client management systems
- Excellent organisational skills with the ability to prioritise and work autonomously
- Understanding of community aged care services, referral pathways and person-centred care principles
- Knowledge of aged care legislation, quality standards and sector reforms
- Current Victorian Driver's Licence and willingness to travel as required
- National Police Check (or willingness to obtain)
