Hope Allied Health Services is adding Specialist Support Coordinators (L3) to the team, fully remote across NSW.
What you'll do
Deliver Level 3 specialist support coordination via telehealth for participants with complex needs. Manage conflict resolution, high-risk planning, crisis response, NDIA reporting, and multi-provider coordination. Keep comprehensive case notes and maintain the participant's plan integrity across stakeholders.
What we're looking for
✔ Relevant allied health / social work qualification (degree-level), AHPRA or AASW registration where applicable
✔ Demonstrated experience at L3 / Specialist Support Coordination level, including complex case management
SDA/SIL experience and community languages (Farsi, Arabic, Mandarin) are a bonus — not a requirement.
