We are seeking a compassionate and reliable Community Worker to provide in-home and community-based support across Brisbane and Surrounds (Ipswich/Springfield, Wynnum, Nothside).

About the Role
This role involves providing practical, person-centred support to individuals with disability and psychosocial needs within their homes and local community.
You will assist participants to strengthen life skills, maintain safe living environments and access meaningful activities that promote wellbeing and inclusion.
Casual / Part-time Roles Available , can offer 38 hours a week.
Key Responsibilities
- Providing structured in-home support with daily living tasks and routines
- Supporting individuals with psychosocial disabilities, mental health, substance use or transitioning from incarceration.
- Supporting skill development and independence within the home environment
- Assisting with community access, appointments and social participation
- Implementing individual support plans and behaviour support strategies
- Monitoring wellbeing and reporting changes in needs
- Liaising respectfully with families and informal supports
- Maintaining accurate documentation and progress notes
About You
- Qualification in Community Services, Social Work, Disability, or related field (or working toward) - desired
- Experience supporting people with disability or psychosocial needs
- Strong communication skills and professional boundaries
- Ability to work independently and follow structured support plans
- Current driver’s licence and reliable vehicle
- NDIS Worker Screening Check (or willingness to obtain)
- Full working rights in Australia
