Metanoia Supports is seeking an experienced and compassionate Family Support Worker to join our growing team. We are looking for a highly organized, motivated and client-focused professional who is passionate about supporting individuals and families to improve their wellbeing, strengthen their independence and access appropriate community services.
This is a full-time position of 38 hours per week.
Key Responsibilities
- Conduct client assessments to identify individual and family support needs and assist in developing tailored support plans.
- Provide practical guidance and ongoing support to individuals and families to achieve their personal and social goals.
- Monitor client progress, maintain accurate case notes and prepare reports as required.
- Assist clients in accessing appropriate community, health, education, housing and support services.
- Liaise with community organisations, government agencies and external service providers to coordinate client support.
- Provide information and education to families regarding available services, community resources and support options.
- Support families in developing independent living skills, strengthening family relationships and improving overall wellbeing.
- Identify barriers affecting clients and assist them in developing practical strategies to overcome social and personal challenges.
- Participate in case reviews and contribute to the continuous improvement of client support programs.
- Maintain professional relationships with stakeholders while ensuring services are delivered in accordance with organisational policies and relevant legislation.
Requirements
- Minimum 1 year of relevant work experience in a similar role.
- Certificate III in Individual Support or a relevant community services qualification.
- Current NDIS Worker Screening Check.
- Current National Police Check.
- Current Blue Card.
- Current First Aid Certificate.
- Excellent communication, organisational and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
Pay: $89,500.00 per year
