What you'll be doing
To provide an effective and efficient Social Work service that addresses the psychosocial aspects of care for patients as identified, providing intervention including support, education and resources to assist in dealing with health events.
To actively participate and contribute to the continued development of the Social Work professional role in the context of Health.
This role will predominantly support longer stay patients that require assistance with NDIS access to ensure a safe discharge home, NCAT (Guardianship) and other complex psychosocial issues that are increasing risk of readmission.
About You
You present a recognised degree in Social Work or an equivalent qualification that provides eligibility for membership with the AASW, grounding your practice in established professional standards. You apply highly developed expertise in Social Work within the Health context, demonstrating a deep understanding of clinical environments, patient needs and the multidisciplinary systems that shape contemporary healthcare delivery.
You excel in completing comprehensive psychosocial assessments and in developing thoughtful, evidence‑based plans and interventions for patients and families experiencing complex presentations. Your work reflects strong clinical reasoning, sound judgement and an ability to navigate sensitive situations with clarity, professionalism and compassion.
You contribute meaningfully to the development, monitoring, implementation and review of the Social Work role within a hospital setting, ensuring that service delivery remains responsive, effective and aligned with organisational expectations. Your commitment to collaborative practice is evident in the way you work alongside multidisciplinary teams and external agencies, particularly NDIS providers and NCAT, strengthening continuity of care and supporting coordinated outcomes for patients.
You communicate with confidence and precision, demonstrating excellence in verbal, written and digital communication. You manage significant workloads with discipline and reliability, consistently maintaining high standards of documentation, time management and professional conduct.
