As a Case Manager in our Home Care team, you'll play a key role in partnering with clients to achieve their goals and remain living safely and confidently at home.
Working as part of a supportive multidisciplinary team, you'll collaborate with Clinical Nurses, Allied Health professionals, Support Workers and fellow Case Managers to deliver personalised care solutions that promote wellbeing, choice and dignity.
About the role
As a Case Manager, you'll coordinate and manage Home Care services for a portfolio of clients, taking a holistic wellness and reablement approach to support independence, health and quality of life.
This is a permanent position, newly created due to the continued growth of our Home Care services. Working Monday to Friday during standard business hours, you'll enjoy the flexibility to balance client visits, working from home and regular attendance at our Port Kennedy office, with a minimum of one day per week based onsite.
Reporting to the Operational Support Manager and working as part of a collaborative multidisciplinary team, you'll be supported by experienced Case Managers, Clinical Nurses, Allied Health professionals and operational teams to deliver outstanding client outcomes.
Key responsibilities include:
- Building trusted relationships with clients and key supports to achieve individual goals and outcomes
- Conducting assessments, care planning and regular reviews using a wellness and reablement approach
- Monitoring Home Care Package budgets, service delivery and referrals in collaboration with clients, colleagues and external stakeholders
- Working closely with Clinical Nurses, Allied Health professionals, Support Workers and broader team members to coordinate safe, high-quality and person-centred care
- Leading, mentoring and supporting a team of Support Workers to deliver exceptional care and services to clients
About you
You're someone who genuinely cares about helping older people live independently and confidently. You combine strong case management skills with empathy, organisation and a collaborative approach to supporting clients, colleagues and stakeholders.
You'll bring:
- Experience in case management, care coordination or a similar role
- Strong understanding of person-centred care practices
- Experience managing client budgets and service delivery
- Excellent communication and relationship-building skills
- Experience leading, supporting and developing Support Workers or other frontline teams
- Strong computer skills across Microsoft Office and client management systems
- A current driver's licence
- The ability to work collaboratively within a multidisciplinary team environment
- You work well with others, value different professional perspectives and contribute positively to team culture.
It would be great if you also have:
- Knowledge of Home Care Package guidelines
- Understanding of government-funded aged care services
- Experience working with culturally diverse communities
- Experience working alongside Clinical Nurses, Allied Health professionals or other multidisciplinary teams
