About Care Cove Support Services
Care Cove Support Services is an NDIS provider committed to delivering person-centred, responsive and high-quality supports to people with disability.
We are seeking an experienced and motivated NDIS Support Coordinator to join our growing team on a part-time basis.
This role would suit someone who has a strong understanding of the NDIS, is confident managing their own participant caseload, and can build positive working relationships with participants, families, allied health professionals and service providers.
About the Role
As an NDIS Support Coordinator, you will work directly with participants to help them understand and implement their NDIS plans, connect with appropriate services and make effective use of their funded supports.
The NDIS describes Support Coordination as helping participants understand and use their plans, connect with NDIS, community and mainstream services, and build their confidence and skills to coordinate their own supports.
Key Responsibilities
- Manage a caseload of NDIS participants requiring Support Coordination.
- Assist participants to understand their NDIS plans, funding categories, budgets and available supports.
- Develop and implement participant support plans in line with their individual goals.
- Identify and connect participants with suitable NDIS providers, allied health professionals, community services and mainstream supports.
- Coordinate services between participants, families, support workers, therapists, plan managers and other stakeholders.
- Monitor participant NDIS budgets and support utilisation to assist with effective use of available funding.
- Obtain quotes, reports and supporting documentation where required.
- Assist participants with establishing and reviewing service agreements.
- Monitor the effectiveness of supports and identify gaps, risks or changes in participant circumstances.
- Support participants through plan reassessments, change-of-circumstances processes and transitions between NDIS plans where required.
- Prepare progress reports, implementation reports and other NDIS documentation.
- Maintain accurate case notes, records and participant documentation.
- Assist with crisis planning and coordinate appropriate services when participants experience changes or challenges in their support environment.
- Advocate for participant choice, control and independence while maintaining professional boundaries.
- Identify, disclose and appropriately manage actual, potential or perceived conflicts of interest.
- Work collaboratively with the Care Cove management and service delivery team.
The role has a significant documentation, reporting and administrative component in addition to participant and stakeholder engagement.
About You
We are looking for someone who is organised, proactive and confident working independently.
The successful candidate will ideally have:
- Previous experience as an NDIS Support Coordinator or in a closely related NDIS role.
- Strong understanding of NDIS plans, funding structures and Support Coordination responsibilities.
- Knowledge of the NDIS participant journey, provider landscape and relevant NDIS processes.
- Excellent verbal and written communication skills.
- Strong report writing and case-note documentation skills.
- Ability to communicate professionally with participants, families, nominees, allied health professionals and other providers.
- Strong organisational and time-management skills.
- Ability to manage multiple participants and competing priorities.
- Confidence working independently with limited supervision.
- Good problem-solving, negotiation and advocacy skills.
- A participant-focused approach that respects choice and control.
- Strong computer literacy and confidence using online systems, email and Microsoft Office.
Qualifications & Requirements
Applicants should have:
- Relevant qualification in Disability, Community Services, Social Work, Allied Health, Mental Health or a related discipline — highly regarded.
- Relevant NDIS Support Coordination experience — strongly preferred.
- NDIS Worker Screening Check.
- Working with Children Check, where applicable.
- Current National Police Check.
- Current Australian driver's licence and reliable access to a vehicle, where participant visits are required.
- Current First Aid/CPR certification — desirable.
- Full Australian working rights.
