About the Role
Join Monash Health Community as an endorsed Nurse Practitioner and contribute to an innovative, person-centred model of care for people living with chronic and complex health conditions.
This is a fixed-term, 12-month opportunity providing parental-leave coverage. Working across the Emergency Department and community-based programs, you will use your advanced and autonomous clinical practice to help prevent avoidable hospital presentations, facilitate earlier discharge and support clients to remain safely and independently at home.
The role combines Emergency Department practice with clinic-based care and assertive community outreach, including home visits across the Monash Health catchment.
In this role, you will:
Undertake comprehensive screening and advanced clinical assessments
Diagnose health conditions and develop evidence-based management plans within scope
Prescribe medicines and order diagnostic investigations in accordance with your endorsement and scope of practice
Identify safe alternatives to Emergency Department presentation or hospital admission
Support timely discharge and effective transitions from hospital to home
Coordinate referrals and access to appropriate health and community services
Deliver health education and empower clients to manage their health conditions
Work collaboratively with clients, carers and multidisciplinary teams
Provide clinical leadership, education and support to colleagues
Contribute to research, quality improvement and service innovation
Clinicians with specialist knowledge or experience in chronic disease management are strongly encouraged to apply.
About You
You are an experienced and motivated Nurse Practitioner who combines advanced clinical expertise with compassionate, person-centred care. You are confident working autonomously across acute and community environments while recognising the importance of collaboration, consultation and timely escalation.
You will bring:
Current registration as a Registered Nurse with AHPRA
Current endorsement as a Nurse Practitioner with the Nursing and Midwifery Board of Australia
Advanced clinical assessment, diagnostic and critical-thinking capability
Experience caring for people with chronic and complex health conditions
The ability to practise autonomously while consulting and collaborating appropriately
Strong communication, relationship-building and conflict-resolution skills
A commitment to evidence-based practice, clinical leadership and innovation
Excellent organisation, time-management and digital capability
A current Victorian driver licence
