The opportunity
We are seeking a full time Social Work Lead. This newly created role will provide advanced clinical leadership, service coordination, and governance for the social work services within Transition Care Services (TCS) located in Adelaide’s CBD and Walkerville.
You will:
- Provide advanced clinical decision making, expert psychosocial assessment, and complex case management for older adults with multifaceted medical, functional, and social needs.
- Lead the coordination of social work services across TCS, ensuring seamless transitions and integrated care pathways.
- Oversee social work practice standards
- Provide supervision and mentoring to Social Workers and others in your team to foster a skilled and confident workforce.
- Drive service development, audit activity, and continuous improvement initiatives that enhance safety, effectiveness, and consumer experience.
- Ensure practice aligns with organisational policies, standards, and best‑practice guidelines, promoting a culture of safety, respect, and continuous improvement.
About you
- Degree in Social Work with membership with the Australian Association of Social Worker (AASW)
- Extensive post‑graduate clinical experience, including advanced practice in gerontology, rehabilitation, acute care, or community health.
- Demonstrated expertise in complex psychosocial assessment, capacity assessment, risk management, and discharge planning.
- Advanced knowledge of geriatric social work, psychosocial assessment, elder abuse frameworks, and transitional care models.
- High‑level skills in complex discharge planning, guardianship/administration processes, and navigating community and aged‑care systems.
- Demonstrated ability to lead teams, influence practice, and drive service improvement.
- Current driver’s licence.
- If successful, willing and able to provide proof of up-to-date vaccinations.
- Holding a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
