Newlight Ability Services is seeking a passionate, dedicated, and experienced Care Coordinator to join our growing team on a full-time basis.
Working Hours: Monday to Friday, 9:00 am – 5:00 pm

About the Role
As a Care Coordinator, you will play a vital role in ensuring participants receive high-quality, person-centred support. You will work closely with participants, families, support workers, and external stakeholders to coordinate services that promote independence, dignity, and choice.
Key Responsibilities
- Coordinate and monitor participant supports and services in line with their goals and needs.
- Build and maintain positive relationships with participants, families, guardians, and stakeholders.
- Develop and implement effective care plans and support strategies.
- Ensure services are delivered in accordance with NDIS Practice Standards and organisational requirements.
- Support and guide support workers to provide safe, quality, and compassionate care.
- Maintain accurate records, documentation, and reporting requirements.
- Promote a culture of respect, accountability, and continuous improvement.
- Adhere to and uphold all Newlight Ability Services policies, procedures, and values.
About You

- Have a strong understanding of the NDIS and person-centred approach to care.
- Be ready and willing to contribute to the ongoing growth and success of Newlight Ability Services.
- Be committed to following organisational policies, procedures, and best practice standards.
Essential Requirements
- Relevant qualifications in Community Services, Disability, Nursing, Social Work, or a related field.
- Solid grasp of NDIS eligibility, pathways, and service models.
- Previous experience in care coordination or the disability sector.
- Current NDIS Worker Screening Check.
- Current Working with Children Check (where applicable).
- Current First Aid and CPR Certificate.
- Valid Australian Driver's Licence and access to a reliable vehicle.
- Strong computer literacy and documentation skills.
