Achieve Something Great
COPILOT is a dynamic and exciting service that provides rapid access to multi-disciplinary geriatric assessment (medical, nursing, and allied health) for older people living in the community setting. Referral enables care to be implemented in the patient’s home environment, avoiding unnecessary transfer to hospital, and leading to improved patient outcomes.
The Ambulance Response Registered Nurse will provide a high quality clinical service to clients aged 16 years and over and their families/carers to optimise health outcomes and support clients keeping well in the community. The Ambulance Response RN will work alongside the COPILOT CNC’s to provide evidence-based care to clients residing in their own homes or nursing homes for acute care based in the community with the goal of avoiding unnecessary transfer to hospital.
Are You the Right Fit?
The right candidate will be confident in working independently as well as within a multidisciplinary team environment and have excellent clinical reasoning and decision making skills, along with excellent communication skills; and have experience across a wide range of settings and clinical areas but ideally will have experience in providing service in Aged Care and community-based settings.
