Our Hearts at Home program is an innovative virtual care service designed to support people living with heart failure in the community. Combining evidence-based clinical care, remote monitoring, exercise prescription, self-management coaching and multidisciplinary care coordination, the program enables patients to receive specialist support from the comfort and safety of their own homes.
Working closely with hospital cardiac services, primary care providers and community-based clinicians, Hearts at Home supports early discharge from hospital, promotes self-management and helps reduce avoidable hospital admissions while delivering person-centred care aligned to each patient's goals and needs.

About the Opportunity
We're seeking an experienced Physiotherapist to join our pioneering Hearts at Home team in Sydney, Melbourne or Brisbane.
This is an exciting opportunity to play a key leadership role in a new and innovative virtual care service supporting people living with heart failure. As a senior clinician, you will provide advanced physiotherapy assessment, exercise prescription and rehabilitation support while contributing to the ongoing development and growth of the program.
Working as part of a multidisciplinary team, you'll combine your expertise in cardiovascular rehabilitation with digital health technologies to deliver contemporary, evidence-based and person-centred care that improves outcomes and enhances quality of life for people living with heart failure.
Key Responsibilities
Deliver high-quality, evidence-based physiotherapy assessments, interventions and exercise prescription for people living with heart failure.
Conduct virtual assessments via phone and video platforms to identify patient needs, priorities and rehabilitation goals.
Utilise digital health technologies to provide safe and effective exercise prescription, monitoring and ongoing clinical support.
Monitor patient outcomes and clinical risk, escalating concerns appropriately and collaborating with the broader care team to ensure optimal care delivery.
Facilitate realistic and meaningful patient-centred goals that support health, wellbeing and self-management.
Collaborate closely with cardiologists, nurses, care coordinators, hospital teams and primary care providers to deliver integrated care.
Support early discharge initiatives and reduce avoidable hospital admissions through proactive intervention and monitoring.
Lead physiotherapy service development initiatives and contribute to the ongoing evolution of the Hearts at Home model of care.
Actively participate in quality improvement activities, service evaluation and program development.
Provide expert physiotherapy advice, mentoring and supervision to colleagues as required.
Maintain accurate clinical documentation, activity reporting and compliance with relevant policies, procedures and regulatory requirements.
Promote the Hearts at Home program and contribute to stakeholder engagement activities.
Participate in orientation, education and professional development activities across the service.
About You
You're a highly experienced Physiotherapist with a passion for cardiovascular care, virtual health and innovative models of service delivery.This role would suit a clinician who combines advanced cardiac rehabilitation expertise with a passion for innovation, virtual care and service development, and who is excited to help shape the future of heart failure care through the Hearts at Home program.
You thrive in collaborative multidisciplinary environments, enjoy influencing service development and are committed to improving outcomes for people living with chronic health conditions.
You will bring:
Current registration as a Physiotherapist with AHPRA.
Minimum five (5) years' post-registration experience, including at least three (3) years in cardiac and/or cardiopulmonary rehabilitation settings.
Highly developed clinical assessment, rehabilitation and exercise prescription skills within cardiac and/or heart failure settings.
Experience delivering assessment and rehabilitation services through telehealth or virtual care platforms (highly regarded).
Strong clinical reasoning and evidence-based decision-making skills.
Demonstrated expertise in interpreting medical management, including diagnostic investigations, treatment plans, medications and biometric observations, and applying this information to physiotherapy assessment, clinical decision-making and exercise prescription.
Excellent communication and stakeholder engagement abilities.
Demonstrated ability to work autonomously while contributing effectively within multidisciplinary teams.
Experience leading service improvement, quality initiatives or program development activities.
Experience supervising, mentoring or supporting the development of other clinicians.
Strong organisational, documentation and time-management skills.
A commitment to patient-centred care and continuous quality improvement.
