About the Team
The Caulfield Hospital team specialises in community-based care, rehabilitation and aged care services. Our services extend across hospital, community and home-based settings, allowing care to be tailored to individual patient needs. Our pharmacists are recognised as essential members of all clinical teams, actively engaging in a unit and team-based care model involving ward rounds and collaborating closely with the multidisciplinary team to achieve excellent patient care outcomes.

About the Role
This is an exciting opportunity for an experienced clinical pharmacist to step into a leadership role within Alfred Health’s Rehabilitation and Acquired Brain Injury (ABI) services. As Senior Clinical Pharmacist - Rehab/ABI, you will provide high-level clinical pharmacy services across a diverse and complex patient group, including those in the Acquired Brain Injury Unit, musculoskeletal and spinal rehabilitation, burns rehabilitation, neurorehabilitation, and related inpatient and outpatient programs.
Working as part of a multidisciplinary team, you will play a pivotal role in optimising medicines use to support patients recovering from injury, trauma, or neurological conditions, often with significant and long-term care needs. You will collaborate closely with Lead and Senior Pharmacists to provide leadership, mentorship, and oversight of clinical pharmacy practice within the team, contributing to service development and patient-centred care.
Our Rehabilitation and ABI services are recognised for their comprehensive, innovative, and highly specialised programs that support patients to maximise independence, recovery, and quality of life. This role offers the chance to shape and influence pharmacy services within this unique and rewarding area of care.

Qualifications & Experience required

Registered pharmacist in Australia

Relevant post-graduate qualification in clinical pharmacy or leadership and management highly desirable

Extensive hospital pharmacy experience, with a demonstrated understanding of contemporary practice in hospital pharmacy services highly desirable

Experience in leading and coaching highly effective team (formally or informally) desirable
