Gentle Care Nursing Services Pty Ltd provides high-quality nursing and support services to clients in the community, including DVA Community Nursing, NDIS participants, aged care clients and private care clients.
We are seeking a compassionate, reliable, and experienced Registered Nurse (RN) to join our growing Community Care team. About the Role
The successful candidate will work closely with clients, families, support workers, allied health providers and other health professionals to ensure clients receive safe, respectful and appropriate care in their own homes.
This role is suitable for an RN who enjoys community-based work, can work independently and has strong communication, clinical judgement and documentation skills.
Key Responsibilities
- Conduct nursing assessments, care reviews and client monitoring.
- Develop, update and review client care plans and clinical documentation.
- Provide clinical guidance, supervision and support to care staff.
- Monitor client health, escalate concerns and report changes appropriately.
- Liaise professionally with families, GPs, allied health providers, support coordinators, case managers and funding bodies.
- Support medication management, wound care, continence care, observations and other nursing tasks within scope of practice.
- Promote safe, respectful and person-centred care in line with organisational policies and clinical standards.
- Assist with compliance, quality improvement and continuous improvement processes.
About You
- Current AHPRA registration as a Registered Nurse.
- Current driver’s licence and access to a reliable vehicle.
- Strong communication, documentation and time-management skills.
- Ability to work independently and as part of a supportive team.
- Compassionate, professional, reliable and respectful attitude.
- Experience in community nursing, aged care, disability, DVA care or home care is highly regarded.
- Current police check, NDIS Worker Screening Check and relevant immunisations will be highly regarded or may be required prior to commencement.
Probation Period
The successful applicant will be employed on an initial three-month probationary period. During this time, Gentle Care Nursing Services will assess the employee's overall suitability for the role, including clinical competence, reliability, communication skills, documentation standards, teamwork, professionalism, and ability to deliver safe, high-quality care within community care settings.
Subject to the successful completion of the probationary period and satisfactory performance, the employee may be offered ongoing employment with Gentle Care Nursing Services.
Visa Sponsorship
For the right candidate, Gentle Care Nursing Services may consider offering Temporary Skill Shortage (Subclass 482) visa sponsorship upon the successful completion of the probationary period. Any sponsorship offered will be subject to the employee's satisfactory performance, eligibility, operational requirements, and compliance with applicable Australian immigration laws and visa requirements.
