The ideal candidate will bring calm, experienced practice in mental health support - particularly psychosis, cognitive impairment and behaviour management - combined with a genuine commitment to person-centred care, community participation and continuity of care.
What You'll Do
Mental Health & Behaviour Support
Support mental health stability through calm, de-escalating communication; recognise and report changes from baseline; monitor for warning signs and escalate early.
Personal Care & Health
Support personal hygiene, grooming, mobility and shower transfers with dignity; follow falls prevention and manual handling guidance; support medication administration and appointment adherence.
Routines & Skill-Building
Establish predictable routines using clear language and visual prompts; support household tasks, shopping, meal planning and community participation through one-step prompting and supported practice.
Documentation & Communication
Record objective observations, context and support provided; complete handover and incident reports promptly; contribute to structured support diaries and behaviour monitoring.
Community Access
Provide 1:1 support for safe community access, transport, appointments, shopping and social participation within an approved plan.
Essential Experience & Skills
- Hands-on experience supporting adults with complex mental health, particularly persistent psychosis, schizoaffective disorder or comparable psychosocial disability.
- Demonstrated de-escalation skills and ability to set respectful, consistent boundaries; calm response to challenging behaviours, suspicion, over-familiarity and verbal reactivity.
- Ability to use simple language, visual prompts, one-step instructions, repetition and supported practice when supporting someone with cognitive impairment.
- Comfortable with personal care (hygiene, grooming), mobility support and shower transfers; willingness to follow falls prevention and manual handling guidance.
- Reliable and able to commit to a stable roster across weekdays, evenings, weekends and inactive overnight shifts.
- Strong observation and documentation skills; able to recognise baseline changes and escalate concerns through appropriate channels.
- Current driving licence and willingness to provide references from relevant roles.
Preferred Attributes
- Experience in Supported Independent Living (SIL), hospital discharge planning or older-adult mental health services.
- Relevant disability support, individual support or mental health qualification (or equivalent demonstrated experience).
- Practical experience implementing behaviour support plans and contributing to structured evidence collection (ABC observations, support diaries).
- Experience in stabilisation after a failed placement or transitional care.
- Warm, patient interpersonal style with interests in gardening, music, games, exercise and community activities.
