What you'll be doing
The Chronic Illness Peer Support (ChIPS) Coordinator provides high level coordination for the ChIPS service. The ChIPS service supports young people aged 12-25yrs who are patients of SCHN and living with a chronic illness and/or disability.
The ChIPS service objectives include:
•Delivering a responsive, peer-led, co-designed peer support service for young people with chronic conditions and/or disability.
•Empowering and promoting the holistic health-care needs of young people, supporting them to optimise their health and wellbeing through self-management, developing independence and enhancing health literacy.
•To foster a safe and supportive community that is accessible and inclusive, where young people are engaged and respected.
•Providing opportunities for young people to use their voice and share their lived experiences.
•Encourage young people to build leadership capacity and support development of platforms for advocacy.
The position will involve liaising with subspecialty clinical nurse consultants, staff specialists and allied health professionals to identify and enrol young people to the ChIPS service, organising and facilitating the ChIPS program events, clinical services and other activities involved with the service.
The Coordinator is required to provide high level, evidence-based psychosocial specialist care within the scope of their profession for young people accessing the ChIPS service. This may include psychosocial screening and assessment, testing, education and intervention.
The coordinator is required to work in partnership with young people and their families and collaborate with treating teams regarding case management issues that may emerge, ensuring high quality ongoing care within a safe environment.
In addition, they will need to appropriately plan for, identify, minimise and manage risks that arise.
This position requires occasional weekend and afterhours work to ensure accessibility to the AYA population.

- Tertiary qualifications in a relevant health-related discipline, current registration with the relevant professional body (e.g. AHPRA or equivalent) and/or demonstrated clinical experience as Social Worker/Art Therapist/Occupational Therapist/Clinical Psychologist/ Diversional Therapist in a health setting.
- Demonstrated enthusiasm and experience in working with adolescents and/or young adults. Experience in facilitating group work is desirable
- Demonstrated knowledge of chronic conditions that affect young people and their impact.
- Demonstrated knowledge and experience in conducting psychosocial assessments and interventions with adolescents and young adults (AYA)
- Excellent communication and interpersonal skills and an ability to adapt these to meet the needs of young people, their families/carers , health professionals and managers
- Demonstrated experience in identifying and managing risks in the context of working with adolescents and young adults in a health setting.
- Ability and willingness to work occasional weekends or evenings to provide a service that is accessible to young people
- Current unrestricted driver’s license
