DH Caring Heart is a community-based disability support provider delivering personalised care, in home assistance and community participation programs across Victoria. We are seeking a dedicated Female Disability /Community Support Worker to join our team and provide high quality support to clients .
About the Role
The successful applicant will work directly with clients to enhance independence, wellbeing and social participation. This position involves:
· Meeting with clients to understand their support needs, daily living requirements and personal goals
· Developing and delivering tailored support plans, independent living skill programs and community access activities
· Monitoring client progress, completing accurate case notes and reporting changes in behaviour, health or risk indicators
· Referring clients to relevant external health, welfare or community services when further assistance is required

Pay: $24.95 – $50.68 per hour
