Join the Lean On Lea Team – Support Coordinator
Lean On Lea is growing, and I am looking for an experienced Support Coordinator to join the team.
This is a fantastic opportunity to work alongside me as I gradually transition towards retirement over the next few years. During this time, you will receive mentoring and support while building your own participant caseload.
About the Role
As a Support Coordinator, you will support NDIS participants to understand and implement their plans, connect with services, build capacity, and achieve their goals. The role requires someone who is compassionate, organised, and confident working independently while maintaining a high standard of documentation.
You will generally meet with up to two participants per day, with appointments typically ranging from 30 minutes to 2 hours. You will also be requiered to completing case notes, reports, referrals, liaising with providers, and other coordination activities.
Essential Requirements
- Previous experience as a Support Coordinator or in a similar NDIS role.
- Sound knowledge of the NDIS, including the legislation, guidelines, and operational framework.
- Excellent communication and interpersonal skills.
- Ability to build genuine, respectful relationships with people living with disability and their families.
- Strong report writing and documentation skills.
- Ability to work autonomously and manage your own workload.
- Excellent organisational and time management skills.
- Current driver's licence and reliable vehicle.
- Current NDIS Worker Screening Check (or ability to obtain).
- Current Working with Children Check (Blue Card) if required.
- Current First Aid and CPR (desirable).

Salary
$45–$50 per hour, plus superannuation (depending on experience).
