About the Role:
Join the Cancer and Ambulatory Services (CAS) Palliative Care Consultation Team as an Advanced Practice Nurse and make a meaningful difference in the lives of patients and their families during some of their most important healthcare journeys.
In this dynamic role, you will provide expert patient assessment, coordinate and triage inpatient referrals, develop and implement comprehensive care plans, and deliver specialist palliative care support across both CAS and the broader hospital environment. As a senior clinical resource, you will mentor and support Registered Nurses within the consultation team while contributing to excellence in patient-centred care.
