NBMLHD Community Mental Health offers a range of specialised services for children, young people, families and older people, through our community-based services and health centres. Our multidisciplinary health professionals and case managers take a collaborative approach to mental health care by linking with their patients to work together and consult within an environment appropriate to the person's needs.
In this rewarding role, you will undertake comprehensive assessments and deliver developmentally appropriate therapeutic interventions designed to support emotional wellbeing and improve mental health outcomes. Working collaboratively with young people and their families, you will help build resilience, strengthen their sense of agency, and enhance their capacity to achieve positive and sustainable wellbeing outcomes.
You will be responsible for developing and coordinating individualised care plans that respond to the unique needs of each young person. Through effective safety planning, care coordination and stakeholder engagement, you will support the delivery of timely and appropriate mental health interventions in the most suitable setting, with a strong focus on providing care as close to home as possible. Building strong partnerships with families, schools, community services and other key stakeholders will be central to your success in this role.
As an integral member of the interdisciplinary team, you will contribute your clinical expertise to support evidence-informed practice and continuous service improvement. You will play a key role in promoting positive mental health outcomes, helping young people navigate their recovery journey, and ensuring they receive holistic, person-centered care that meets their individual needs.
