As our organisation continues to grow, we are seeking an experienced and dedicated Community Worker to join our team. The successful candidate will work closely with clients, families, government agencies, and community organisations to assess individual needs, coordinate appropriate support services, and empower people to achieve greater independence, social inclusion, and an improved quality of life.
Duties and Responsibilities are:
- Identify the social, health, housing, employment, and welfare needs of our clients through assessments, consultations and community engagement activities.
- Assist in development, implementation, and support community development programs that promote social inclusion, wellbeing, and community participation for disadvantage clients.
- Facilitate community meetings, workshops, and forums to encourage collaboration and identify practical solutions to local issues.
- Provide information, guidance, and solutions to clients seeking assistance with housing, healthcare, employment, education, financial support, and other welfare services.
- Advocate on behalf of disadvantaged or vulnerable individuals and groups to ensure they receive equitable access to appropriate community resources and support services.
- Interview clients to assess their needs and develop, implement, and monitor personalised education, training, and support plans to enhance their skills and wellbeing.
- Monitor the effectiveness of support provided and report on clients' progress and outcomes.
- Coordinate community projects and outreach initiatives aimed at improving the quality of life and addressing identified community needs.
- Assist in planning and delivering recreational, educational, cultural, and wellbeing activities that strengthen community engagement and social cohesion for the clients.
- Provide referrals to relevant agencies and organisations to address clients' ongoing support needs
- Establish and maintain strong relationships with community members, government agencies, service providers, and non-government organisations to improve access to essential services.
- Monitor and evaluate the effectiveness of community programs and recommend improvements based on participant feedback and identified outcomes.
- Conduct community needs assessments and collect relevant data to support the development of new programs and funding proposals to the concerned agencies.
- Promote awareness of available community services, government programs, and support initiatives through information sessions, presentations, and community events.
- Maintain accurate case notes, program records, reports, and confidential client documentation in accordance with organisational policies and legislative requirements.
- Assist clients in developing life skills, independence, and confidence through mentoring, education, and ongoing support.
- Ensure all services are delivered in accordance with relevant legislation, organisational policies, ethical standards, and workplace health and safety requirements.

Qualifications Required: To be eligible for the above position, the candidate should have Associate Degree, Advanced Diploma or Diploma OR at least three years of full-time, relevant experience in lieu of formal qualifications
Work Experience: At least one year of full-time related experience.
Remuneration: The appropriate candidate will be paid $90,000 annually plus superannuation.
Other employment entitlements will be as per National Employment Standards and as per Fair work Instruments as prescribed by the Australian Government.

Pay: $85,000.00 – $95,000.00 per year
