Position Title
Disability Support Worker
Reports To
Team Leader / Service Manager
Position Purpose
The Disability Support Worker provides person-centred support to people living with disability, assisting them to achieve their goals, maintain independence, participate in their community, and improve their quality of life in accordance with their NDIS plan.
Key Responsibilities
Personal Care
Assist participants with showering, dressing, grooming, toileting, and personal hygiene.
Support participants with mobility, transfers, and use of mobility equipment.
Assist with meal preparation and feeding where required.
Daily Living Support
Support participants with household tasks including cleaning, laundry, shopping, and meal preparation.
Assist participants to develop independent living skills.
Promote participant choice, dignity, and independence.
Community Access
Support participants to attend appointments, social activities, recreational activities, and community events.
Assist participants with transportation when required.
Encourage social inclusion and community participation.
Health and Wellbeing
Monitor participant wellbeing and report any concerns.
Assist with medication prompting and medication administration in accordance with organisational policies and training.
Respond appropriately to incidents, emergencies, and health concerns.
Documentation and Reporting
Complete accurate progress notes and shift reports.
Report incidents, hazards, injuries, and changes in participant needs.
Maintain confidentiality and privacy of participant information.
Behaviour Support
Follow Behaviour Support Plans and Risk Assessments.
Use positive behaviour support strategies.
Support participants with complex needs in a respectful and professional manner.
Essential Requirements
NDIS Worker Screening Check.
Working With Children Check (where required).
Current First Aid and CPR Certificate.
Current Driver's Licence and reliable vehicle.
National Police Check.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
