What you'll be doing
To provide safe, compassionate and high-quality person-centred assessment and case coordination to clients.
Ensuring care is delivered in collaboration with clients, their families/carers and a multidisciplinary team to achieve identified goals and outcomes in line with MNCLHD Aged Care Assessment Service guidelines (ACAS).
About You
You describe yourself as a professional grounded in qualifications recognised across key Australian health and social care boards, bringing the credibility and readiness required to practise as a social worker, occupational therapist, physiotherapist, psychologist or registered nurse. You emphasise your ability to carry out comprehensive aged‑care assessments, approaching complex cases with a problem‑solving mindset that prioritises positive outcomes for clients and their families while maintaining a genuinely person‑centred focus within a multidisciplinary team. You convey confidence in your experience across acute and community settings, supported by a strong understanding of reablement, rehabilitation, geriatric conditions and chronic disease management. You highlight your clinical depth in caring for older adults with multifaceted needs, including dementia and end‑of‑life care, and you show an awareness of the sensitivity and skill required in these contexts. Communication is clearly one of your strengths, allowing you to identify key issues, collaborate effectively with service providers, GPs and other health professionals, and build trust with clients and carers. You also note your solid computer skills and ease with a range of software programs, supporting accurate documentation and efficient workflow. Finally, you acknowledge the practical requirement of holding an unrestricted Australian driver’s licence, ready to travel as needed to meet the demands of community‑based care.
