We are seeking a TAC/NDIS Support Worker to work in a casual (8 to 24 hours/fortnight) capacity based at KANIVA.
ABOUT THE ROLE
- The Support Worker provides in-home and community-based supports to NDIS participants and TAC clients to enable them to maintain and/or increase their independence, safety,
participation, and wellbeing.
- The role is delivered in a person-centred, rights-based manner, consistent with the NDIS Practice Standards, NDIS Code of Conduct, TAC requirements, and West Wimmera Health Service (WWHS) policies and values.
KEY SELECTION CRITERIA
Essential:
- Demonstrated effective communication and organisational skills.
- Commitment to person‑centred practice, empowerment, inclusion, and participant choice and control.
- Ability to work independently and as part of a multidisciplinary team.
- Current First Aid Certificate (including CPR), or willingness to obtain.
Desirable:
- Certificate III or higher in:
- Individual Support
- Disability
- Aged Care
- Community Services
- or a related field
- Previous experience supporting people with disability, injury, or ageing in community or residential settings.
- Experience working within NDIS and/or TAC funded services
PRE-REQUISITES:
- NDIS Worker Screening Check.
- Working with Children Check.
- Evidence of required vaccinations (including influenza) as per Department of Health directives.
- Current Australian Driver’s License.
- Ability to travel between client locations.
- Apply First Aid.
KEY RESPONSBILITIES:
- Provide support in accordance with each client or participant’s Care Plan, support plan, and funded supports.
- Support people to exercise choice and control, maintain dignity, and work toward their individual goals.
- Assist clients/participants to live safely and meaningfully within their home and community environments.
