About the Opportunity
Step into a rewarding role as an Integrated Care Coordinator and help improve outcomes for people living with chronic and complex health conditions across the Corowa region.
As a Registered Nurse, you'll coordinate person-centred care across hospitals, general practice, community health and social support services, working closely with patients, families and multidisciplinary teams to deliver seamless, integrated care.
About You
Our ideal candidate will demonstrate:

Current registration as a Registered Nurse with AHPRA and completion of a Graduate Nurse Program or equivalent.

Experience in comprehensive assessment, care planning and coordinating care for people with chronic or complex health needs.

The ability to work independently while collaborating effectively within multidisciplinary teams and across healthcare services.

Excellent communication, organisational and relationship-building skills, with a commitment to patient-centred care.

A sound understanding of quality, safety and continuous improvement, with a reflective approach to professional practice.

A current NSW Driver Licence and willingness to travel across the local service area as required.
