An opportunity is available for a Registered Nurse (sole) to join the Better Health Network Community Health Nursing team at Launch Housing Southbank Crisis Accommodation.
The Community Health Nurse works within a nurse-led primary healthcare service supporting people experiencing homelessness, many of whom have complex health and social needs. Working within harm reduction, trauma-informed and person-centred frameworks, the role works alongside another BHN Community Health Nurse and closely with Launch Housing staff, BHN services, the onsite GP clinic, and external health and community providers.
What would I be doing?
- Comprehensive health assessment and care planning
- Primary healthcare, health education, and health promotion
- Immunisation and pathology collection
- Wound care, diabetes management, chronic disease support, venepuncture, pathology follow-up and Naloxone supervision
- Care coordination, referral, advocacy, and supporting access to health services
- Responding to acute health concerns and onsite health emergencies
- Recognising clinical deterioration, escalating care appropriately, and supporting emergency, resuscitation, trauma and overdose responses when required
- Providing health information and secondary consultation to Launch Housing staff
We are looking for a Registered Nurse who:
- Holds current registration with AHPRA as a Registered Nurse
- Has at least five years of post-registration nursing experience
- Accreditation as a nurse immuniser or working toward
- Is confident working autonomously and managing a varied clinical workload
- Has demonstrated experience in a high-acuity clinical environment, such as emergency, urgent care, general practice, primary care or an equivalent acute care setting
- Has strong clinical assessment, communication, and care coordination skills
- Is confident responding to clinical risk, mental health concerns, alcohol and other drug-related presentations, and situations requiring de-escalation within safe practice parameters
- Can work effectively with people experiencing complex health and social needs
- Has an understanding of, or interest in, homelessness and the health inequities experienced by people without stable housing
- Works collaboratively within a multidisciplinary and interagency environment
Experience in community health, primary care, emergency nursing, mental health, alcohol and other drugs, sexual health, wound care, or working with people experiencing homelessness would be highly regarded.
Location
This role is primarily based at Launch Housing Southbank, Melbourne.
