About the Role
We are seeking caring and reliable Home Care Workers to support Transitional Care clients living in the Brisbane South / Carindale region. These clients are typically transitioning from hospital back to home and require consistent, time‑sensitive support — especially early in the morning.
Key Responsibilities
- Domestic assistance — cleaning, laundry, meal prep
- Personal care — showering, grooming, toileting
- Social support and companionship
- Transport support — appointments, shopping, community activities
- Driving between clients across the Brisbane South / Carindale area
- Supporting Transitional Care clients with structured routines and recovery‑focused goals
What You Need
- Certificate III in Individual Support (Ageing/Disability) or equivalent
- A caring, reliable, and professional approach
- Strong communication skills with clients, families, and the care team
- Patience, empathy, and a genuine commitment to improving seniors’ wellbeing
- Current National Police Check (or willingness to obtain)
- Confident working independently
- Current First Aid & CPR (or willingness to obtain)
- Open Australian Driver Licence
- Reliable, registered vehicle with comprehensive insurance
- Willingness to travel between clients
- Permanent Australian Residency or valid work rights
- Early‑morning availability (essential for Transitional Care clients)

Pay: $43.00 – $44.00 per hour
Licence/Certification:
- Certificate III in Individual Support or equivalent (Required)

- Reliable, registered vehicle with comprehensive insurance (Required)
