The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people’s homes, other community venues as well as virtual care (telehealth).
We are seeking a dedicated and compassionate Child and Family Health Nurse to provide family-centred care and support to families with children aged 0-5 years. In this rewarding community-based role, you will deliver Universal Health Home Visits, developmental and health surveillance, screening, and the early identification of health, developmental and family vulnerabilities.
Working collaboratively within a multidisciplinary team, you will promote positive health and wellbeing outcomes through evidence-based practice, timely intervention, advocacy and culturally responsive care. You will also contribute to a supportive learning environment by providing clinical guidance, mentoring and preceptorship to staff and students.
This role is ideal for a nurse who is passionate about early childhood health, enjoys building strong relationships with families and is committed to delivering high-quality care that helps children thrive.
