About Mercy Homecare Services
Mercy Homecare Services is a registered NDIS provider supporting people with complex, high-intensity needs across Australia. We specialise in the participants other providers find too hard, high-need, high-risk, and complex care because we believe everyone deserves support delivered with skill, consistency, and genuine respect.

About the Role
We are looking for an experienced Support Coordinator to join our team and take on an established caseload of participants with complex and high-intensity support needs. This is a meaningful role with real impact where you will help people understand their NDIS plans, connect them to the right supports, and build the capacity and stability that genuinely changes lives. You will be trusted with autonomy, backed by clear systems and leadership, and supported to do the work properly, not rushed through unrealistic caseloads.
What You'll Be Doing
- Coordinating supports for participants with complex and high-intensity needs
- Helping participants understand and implement their NDIS plans
- Connecting participants with the right providers and community supports
- Building participant capacity, independence, and confidence to navigate the scheme
- Monitoring plan budgets and ensuring supports stay aligned to funding
- Navigating complex and sometimes high-risk situations with sound judgement
- Liaising with the NDIA, Local Area Coordinators, families, and stakeholders
- Maintaining clear, accurate documentation and progress reporting
What We're Looking For
- Required: Experience in Support Coordination, or a related field (allied health, social work, community services, case management)
- Required: Strong working knowledge of the NDIS including plans, funding categories, and the support landscape
- Required: Confidence working with complex, high-need participants and challenging situations
- Required: Excellent communication, relationship-building, and documentation skills
- Required: Current NDIS Worker Screening Check and Working with Children Check (or willingness to obtain)
- Required: Current driver's licence and reliable transport
- Highly regarded: Experience with high-intensity supports, SIL, or specialist support coordination
- Highly regarded: A relevant qualification in social work, allied health, or community services
