The Central Coast Local Health District (CCLHD) is seeking enthusiastic, passionate, and compassionate Registered Nurses to join the Aged Care Response Team in permanent full-time and permanent part-time positions.
This is an exciting opportunity for motivated nurses who are looking to expand their clinical skills and knowledge while making a meaningful difference in the lives of older people. Working as part of a multidisciplinary team, you will provide expert, person-centred care to residents of Residential Aged Care Facilities, helping to prevent avoidable hospital presentations, promote quality of life, and support residents to receive the right care in the right place.
About the Opportunity
The role involves providing advanced clinical assessment, expert review, education, and evidence-based recommendations to Residential Aged Care Facilities (RACFs) to support the prevention of avoidable emergency department presentations and hospital admissions. This is achieved through comprehensive nursing assessment, care planning, implementation, and evaluation, using best-practice principles within a collaborative, multidisciplinary framework. Strong partnerships are fostered with residents, families and carers, RACF staff, general practitioners, and other key stakeholders to ensure person-centred, timely, and coordinated care.
Community nursing focuses on promoting and maintaining health while supporting early recognition of clinical deterioration, timely intervention, rehabilitation, and chronic disease management. These interventions aim to prevent illness where possible, slow disease progression, minimise the impact of existing health conditions, maximise independence and quality of life, and provide compassionate end-of-life care when appropriate.
Recognising the significant influence of social, environmental, and individual factors on health outcomes, Community Nurses deliver responsive, flexible, and person-centred care that adapts to the evolving needs of the community. A key component of the role is strengthening the capability of Residential Aged Care Facilities through clinical leadership, education, and proactive care planning, enabling residents to receive appropriate care in place and reducing avoidable transfers to emergency departments.

About You
You will bring a strong commitment to high-quality community care and a passion for helping people maintain their health, independence, and dignity. You will have:
- Registered Nurse (Division 1) with AHPRA, with current registration.
- Recent and extensive nursing experience in Acute Care or Community Nursing.
- Proven experience using advanced clinical skills in assessment and care planning
- Sound computer and keyboard skills with computer literacy, or willing to be trained in computer use relating to currently used computer systems within the local health district
- Current valid Australian Drivers Licence
