About the Opportunity
We are seeking an experienced and motivated Aged Care Nurse Practitioner/ Transitional Nurse practitioner to join our growing multidisciplinary team.
In this advanced practice role, you will provide comprehensive assessment, diagnosis, treatment and ongoing management for older adults living with complex and chronic health conditions. Working across residential aged care, outpatient clinics, community settings and the newly established Aged Care Outreach Service, you'll help improve quality of life, reduce avoidable hospital admissions and support people to receive care closer to home.
As the Aged Care Nurse Practitioner, you will:

Provide advanced assessment, diagnosis, treatment and management for older adults with complex health needs.

Deliver evidence-based care while building collaborative partnerships across residential aged care, outpatient clinics, homes and community settings.

Prescribe medications and implement therapeutic interventions within your endorsed scope of practice.

Lead clinical decision-making and collaborate with multidisciplinary teams to deliver person-centred care.

Support innovative models of care, including the Aged Care Outreach Service

Provide clinical leadership, education and mentoring while contributing to research, quality improvement and service development.

Our ideal candidate will demonstrate:

Authority to practice as a Nurse Practitioner with the Nursing and Midwifery Board of Australia (AHPRA), or demonstrated evidence of eligibility to obtain authorisation within 3 years.

Extensive experience caring for older people with complex and chronic health conditions.

Advanced clinical assessment, diagnostic reasoning and prescribing skills.

Ability to work both autonomously and collaboratively within multidisciplinary teams.
-
Experience leading clinical practice, quality improvement and service innovation.
-
Excellent communication, leadership and stakeholder engagement skills.
-
Commitment to person-centred care, continuous professional development and evidence-based practice.

Current Class C Driver Licence and willingness to travel across the district.
