Superior Home Health Services is looking for a caring, confident, and dependable Home and Community Registered Nurse Care Partner to support our North Brisbane clients. You’ll be based in our friendly Brisbane Office with 2 days per week out visiting clients. We would also be open to being based on the Sunshine Coast. Company vehicle available for all business travel.
This is a full-time, 38 hours per week, role, however 0.8 FTE would be considered.
Responsibilities will include:
- Working within a person-centered, enabling environment that respects the rights and needs of each client.
- Manage case management for a variety of aged and disability clients;
- Capped client loads of 45, with admin support;
- Undertake client intake assessments;
- Manage government funding administrative tasks;
- Assisting clients with funding utilisation maximisation;
- Assisting clients with medications and support;
- Provide client and support worker education where applicable;
- Observe, care and report any changes in the health and wellbeing of the client;
- Undertake skilled nursing tasks where required;
- Responding to emergency situations
Essential Criteria:
- Bachelor of Nursing - must be able to do continence, falls, dementia assessments
- Excellent verbal and written communication skills
- Willingness to learn, develop and take on new challenges
- Current National Police Check
- Current First Aid & CPR certificates
- Current unrestricted driver’s license
- NDIS Worker Screening including Blue Card (or equivalent)
- Community experience, 2 year minimum
Preferred Criteria
- Previous Care Coordination/Case Management in Community
- Post Grad qualifications and specialties relevant to community care
- Located Brisbane - North Lakes

Pay: $105,500.00 – $110,000.00 per year
