NBMLHD Community Mental Health offers a range of specialised services for children, young people, families and older people, through our community-based services and health centers. Our multidisciplinary health professionals and case managers take a collaborative approach to mental health care by linking with their patients to work together and consult within an environment appropriate to the person's needs.
This is a senior social work role for someone who knows that young people’s mental health is shaped as much by systems and circumstance as by symptoms. Within the Youth Mental Health Initiative, you will deliver advanced, evidence informed assessment and intervention with a strong focus on risk, family inclusive practice and the social determinants of mental health. This position will work across Headspace Penrith and Headspace Hawkesbury Offices.
You will work alongside young people aged within headspace services across Penrith and Hawkesbury, and with their families and care networks, to enhance safety, stability and sustainable recovery. A core part of this role is leading complex care coordination and system navigation, supporting access to NDIA related supports, income and welfare assistance, housing and homelessness services, education pathways, youth and family agencies and a wide range of government and non-government providers.
