Elouera Home Support is currently seeking a compassionate and experienced Registered Nurse (RN) to join our growing team delivering in-home nursing and community care services.
This role is ideal for nurses with experience in:
- Community Nursing
- Home Care
- Aged Care Nursing
- Disability Support (NDIS)
- Chronic Disease Management
As a Home Care Registered Nurse, you will provide clinical care and health support to clients in their homes, helping them maintain independence, dignity, and quality of life.
We offer flexible casual shifts, allowing you to build hours around your availability while making a meaningful difference in the community.
About the Role
As a Community Registered Nurse, you will deliver clinical care and health support to clients in their homes and within the community.
Your responsibilities may include:
- Conducting health and wellbeing assessments
- Developing and reviewing individualised care plans
- Medication management and administration
- Wound care and clinical treatments
- Monitoring chronic conditions and health changes
- Supporting hospital discharge transitions
- Providing education and guidance to clients and families
- Liaising with GPs, allied health professionals, and care coordinators
- Maintaining accurate clinical documentation and progress notes
- Supporting safe, person-centred care in the community
This role involves working independently in clients’ homes while maintaining strong communication with the office and care coordination team.
What We’re Looking For
We are seeking Registered Nurses who are compassionate, reliable, and confident working independently in community settings.
Ideal candidates will have:
- A genuine passion for supporting people in their homes
- Strong clinical assessment skills
- Excellent communication and documentation skills
- Confidence working independently in community environments
- Professional judgement and critical thinking
- Reliability and responsiveness to team communication
- Flexibility to travel between client homes
Experience in community nursing, aged care, disability services, or home care is highly regarded.
Minimum Requirements
To be considered for this role you must have:
- Current AHPRA Registration as a Registered Nurse
- Nursing Qualifications
- Current National Police Check
- Working With Children Check (NSW)
- Current First Aid & CPR certification
- Valid Driver’s Licence
- Access to a reliable vehicle with comprehensive insurance
- Able to cover suburbs such as Balmain, Lewisham, Dulwich Hill, Summer Hill and Rozelle etc
Experience in home care, aged care, NDIS services, or community nursing will be highly regarded.
