Your role
Provide participants, their family and carers with information and support about the Scheme and the Agency.
Assess participant needs and create and amend support plans including making decisions (or recommendations to the Manager) regarding the necessary and reasonable treatment, care and support needs of participants.
Understand and discharge delegated functions in accordance with the Act and follow Agency policies and procedures.
Identify ways to empower participants in the development, implementation and monitoring of support plans, to reflect participant goals, choice and control, and measurable outcomes.
Supported to identify effective options for treatment, care and support, balancing the needs of participants with Scheme requirements, sustainability, and community expectations.
Manage service requests, approvals and payments in accordance with agreed needs and assessments and plans.
Liaise with legal representatives of participants and their families/carers as needed.
Empower participants to use tools and resources available to maximise independence, participation in the community and employment.
Liaise and collaborate with stakeholders to ensure successful implementation and maintenance of support plans.
Participate in a learning culture by sharing information, keeping abreast with current best practice.

Knowledge and understanding of support planning principles and processes.
Ability to manage support planning and goal-setting processes whilst discharging delegated functions in accordance with the Act.
Well-developed organisational skills and the ability to lead self and/or others within a dynamic environment, displaying a collaborative, autonomous, flexible and agile working style and approach.
Well-developed communication skills, ability to build rapport, and negotiate with a variety of internal and external stakeholders.
Ability to make sound decisions and recommendations based on relevant evidence in relation to disability services.
